As long as they know what's going on, with a reasonable lead time, that's all anyone really wants. We all get that life happens, people get sick, die, cars blow up, water heaters leak and flood basements, whatever. But when you've got something set up, and that life thing happens, blast out a text/e-mail/whatever, just so people aren't standing around with (literally) their dicks in their hands, expecting you to show up and help them with that.

the night before the morning appointment that she knew wasn't happening, I suspect he'd have been disappointed, perhaps pissed, but not vindictive.
As it was, without commenting further on his actions, hers were fucking rude.
Your rates are your rates, and as long as you're clear about them, people can choose to see you or not. If they want to bitch and cry about, "Well, you charge $50 less in Longview!", you tell them, "You're right! Since the convenience of me being in your hometown isn't worth it, you come see me for the regular rate at my place. When will you be in Longview, by the way? I'd hate to miss you!"
