Tell your doc why you want to be tested. Especially if you have a SO and you share the same doctor, remind the doc you'd like this confidential. There might be a problem if you test positive as the doc may then have a duty to tell the SO. I don't have that problem, but if I did, I'd visit with my doc in advance.
