Yeah, first time I heard that word it was from a guy in my fraternity. He was in the habit of calling everyone "Smeg". I finally asked him what it meant and initially thought he was just pulling my leg!

Actually there are a couple of medical conditions that may be contributing to his problem. The first, phimosis, can affect a small population of adult men. Think of it as a tight foreskin. Normally it disappears by the time a boy is 2-3 or so, but occasionally sticks around. The other condition is balanitis, which often comes from a few drops of urine remaining at the tip of the penis after using the restroom. It can actually cause the head to stick to the foreskin and cause pain when it's pulled back. It may not necessarily be a hygiene issue so much as a physiological issue. Either way, some steroid creams or possibly even surgery (circumcision) may be called for. On a side note, people think that most men are circumcised. That's actually not true. In the US, we average about half or so (roughly 55%), but it varies widely state to state.
