Do you prefer a lived-in incall or one that's clearly business?

If the place is clean, or cleanish, and doesn't stink, then I don't care. To me it's whatever makes the provider more comfortable. If it's in a place where she can feel safe and relaxed, then I know that we will more than likely have a better time together.
I think if your incall is an apartment it should look/give off the feel that it is lived in...Why? ...Because that way when maintenance men or other like fire marshalls who do yearly checks come in to your place it doesn't ring suspicion vs the gilrs whos apts are business only.. nothing on the walls..no personality. .just an bed and not much furniture
I don't think you should play where you rest your head... When I am out of town I often get another place just to sleep. Of course this is not every time... But I think there are many reasons why you shouldn't make your residence your playhouse...
To each their own...
