I go into a session with the donation previously agreed to in my front pocket, and then a few extra ($40-60) in my wallet. If the session is so good I want to tip her, I pull the tip from my wallet and add it to the donation while she's watching, so she knows specifically that she's being tipped (though I don't explicitly say so).

If I miscounted the donation and left more than I intended, it would be hers (my stupidity isn't her fault). I probably won't even realize it, but if I do I figure she will put it to good use, and I'm sure she won't feel bad about it. It might even make her more eager to see me the next time.
