@asusr10, It is required by law and enforced through compliance visits for them to have filled out proper paperwork for all visitors. When I helped set this up for a friend, she was stressed that it had to be done - because she told me that clients did not want their names and phone numbers available on the paperwork, yet she had a Texas compliance officer coming back to check and see that she was following procedure. So I ask this forum: If you do not care that you name and phone number is on a piece of paper in the office, volunteer to fill it out at your place of choice so that they are able to maintain records to meet compliance

The subamps are supposed to collect a signed paper form with type of massage and consent for the massage. Something like this

Vigilant17 is correct on the paperwork. Fines start at $1000.00 for 1st offense and can lead to loss of Massage Establishment License. They don't care what name, phone#, or address you put. They are not verifying the information only need to have it on file for TDLR, and it's only required on the first visit.

so we're talking about two different things! Yoyo was popped on a city inspection for COA. This form you are talking about is a state form from TDLR. This form has nothing to do with what happened at Yoyo.
