Art work may vary the cost. But most template sites take less than 24/48 to setup a working site. Tweaks can always be added.

You can host you own site for less than 10.00 a month.
Custom(non template) sites can cost a little more but normally one can work in most budget ranges and should never really exceed 500(escort style few pages).

I don't think hobbiers are a fan of online forms that require personal details; because most of the information is stored and most folks would not know how to manage that stored data and tend also still pass on info in unsecured manners or leave on the hosted server which can be accessed.
