Are the gals in South Texas not requiring screening? If they do, what kind of information are they asking for?

I don't have p411 so for me usually a description of myself (Age, race, and a few other details depending on the provider) and if they need a reference I'll usually tell her to look at the providers I have reviewed for a reference .
