It looks like a moderator tried to link it to a provider's profile, but they chose the wrong one:

When it should be:

This was likely due to a (temporary) name change of the provider in question. She was initially calling herself Macy, then briefly started using Karmen when I did the review, then went back to Macy.
Normally I wouldn't bother asking for fixing something this old but I see Macy's back providing and seems to be taking a more active role here now. Can a mod fix this? I considered using the report button but that seems inappropriate to use to request a tech fix.
