How do I write a review? Do I need premium access to do this?

No, you don't need to have Premium Access, but if you write a review that meets the criteria you should receive six weeks of Premium Access.
Go to the Independent Reviews or Agency Reviews forum in the city that the appointment took place, hit the New Review button, fill in the form and hit Submit. The link that cckid2006 provided has excellent advice on what should go into the review.
