Filing sex report in Texas

To enforce efficiently the new low prohibiting abortion after 20 weeks,
Texas government needs to know exactly the date from which to calculate these 20 weeks.
For that purpose, after the law takes effect on Sep. 1, every Texas resident is required to file a monthly report explaining when, where and with whom he (she) had sex. The report needs to be filed on the standard form S-69ex. This form also conveniently includes check-boxes to state whether the sex was vaginal or otherwise, and if a condom was used. You can download one from www.texas.gov
