Attempting to reinstate verification

I'm a provider in the Denver/ Colorado Springs area. I was active on the site in 2012-2013, but was deactivated due to inactivity. Since returning I have emailed the assigned moderators with my info, webpage, and reviews but have yet to be reverified. How do I find out whether or not my request has been accepted/rejected?
