I was once a verified provider I was off the site for a period of time I am not verified anymore how do I go about being a verified provider again??

Could one of the mods respond to this? I personally would like to know the answer, as I'm sure the OP would as well.

Providers who let there account go inactive after 45 days will have to resubmit proof of age again. She was sent the information on how to accomplish this.
