Review Template

I have searched for a template to leave reviews but cannot locate. Any help would be great

Top left of any encounter forum. Tab says new encounter

click on 'Encounter Reports' and then in the middle of the screen you will see a blue rectangular box that says 'New Encounter Report' and then fill in as much info as you can

If it was a snake, it would have bit me.
