I can't figure out how to post a review for a provider. Can someone assist?

Go to the area you are from, click on the "Independent Provider Review" forum, click the "New Review" button 
This will bring up a template for the review. Fill out the info and click "submit."

You may want to first compose the ROS part of the review offline in Notepad or Word or something else similar. Then when you go online to post the review copy and paste the composed ROS.
