Encounter Reports Rejected - No Contact Info

My Encounter Reports are being rejected or disqualified due to lack of contact info. Can you please help me understand what contact info is supposed to be included in an encounter report?

I peeked at the last 3. Honestly it looks like you half-ass your reports.

One had no phone number, another had no activities, and the 3rd had contradicting names of the Provider.

That's just your last 3, God knows what the other 4 reveal.
