Here's a higher res of the full ad the shoe one came from. You're looking at two heels right next to each other (imaging a woman standing with her legs together); the strap goes around the left ankle one way, and the opposite way on the right.

Bodyparts, like an arm or a leg can look like some other body part, like ass or boobs depending on the angle.
