This is the third review of yours I’ve seen that has no ROS. You’re doing these ladies a disservice by not explaining why you are recommending them to others.
Plus, if you’re trying to get Premium Access, you have to provide all the information in the template, including ROS.

One thing that hasn't changed is having multiple profiles for the same provider. Here, we have a provider who has a profile with the same phone nubmer as in the review

attached to another profile, perhaps of the same provider, but said profile has a different phone number and fewer reviews.
It'd make it a little less confusing if whoever does the assigning has some basic knowledge of the area from which the review originated, or at least be able to recognize longtime providers in that area.
