My advice (if you go to the doc) would be to say that you've been doing this with a boyfriend or significant other and developed the injury. With the newly mandated computerized medical records for national healthcare, I wouldn't trust anything that goes in to your medical record to remain safely confidential. I just don't trust the government enough to advise you to tell the honest truth in your situation. The government has proven time and time again to be asleep at the switch when it comes to protecting privacy (remember the case a few years ago where a VA employee had a laptop stolen that contained personal health information on thousands of veterans?) To limit the number of people that see your medical record, you could also pay the full cost of the office visit in cash (ask if the office offers a cash discount, most walk-in care clinics charge $95 or less) and then not submit an insurance claim.

You may find some "Doc in a Box" who only runs people in and out, but if this is bothering you, something could be torn or infected down there. Find someone you trust. From your note, I gather this has gone on for some time. If it was in the last couple of weeks, you could just watch things for a while.
