trying to get verified as a provider

I am trying to et verified as a provider, and followed all the instructions and sent the email with the required information almost 3 weeks ago, and still am not verified. What else do I have to do? The email has not even been opened yet. I am in the Erie PA area. This is frustrating and I am getting ready to give up.
