profile and Jendoll

How do I edit my profile? Also, is there a way to completely delete Jenndoll, someone else made that and I had no control over it and I want it off there. I am an indepedant provider now so Jayden76 is my new name and info. can you check my status and change me to a provider as well. thank you Jayden
