For the TL;DR rundown, this app is called Carrier IQ (CIQ). It can be installed on many devices although most notably, android. Carriers proven to contain it are Sprint, T-Mobile, and AT&T. Also note that while Verizon claims to not have this, they do still record everything you do and sell it off. I would highly recommend reading up on this.
