I'm a little new to hobbying and I've been to a couple AMP's so far. I know that in the Dallas AMP's, 30 minutes is $140 and 60 minutes is $200 which includes tip. For the independent providers, if they have a listed rate of $250, does that include tip or are they looking for a tip on top of that rate?

At an AMP the tip is fixed, but there's nothing stopping you from dropping a little extra except the wrath of the entire readership on ECCIE.
Agency girls would, in theory, be a gray area. But in practice it's not. Someone else might have an opinion on whether gifts or other non-monetary appreciation is more common.
