How many bans can one go through before being banned permanently?

Say a member were to take a break and not log in for quite some time....( 3 months or more )
Gentlemen: would he have to reset his account? regain access in some way? or simply log back in with no issues?
Ladies: do they have to reset and verify themselves again? Or do they also retain access regardless of the time span between logins.
I ask because I'm nebby. Plain and simple.
No seriously, I've noticed some websites now require one to log in on a regular basis or risk having to re-verify themselves.
