How do I get my Verified Provider status back?

This is Aliesha... Otherwise known in the DFW area as Aliesha the freak or freakaliesha... I have not checked in in quite a long time but recently have and learned that I lost my Verified Provider status... I would desperately like to regain that... Please let me know what steps I must take in order to do so please Eccie team someone help me
