Encounter format!

Apparently the format used for encounter reports is not standardized for all regions! Upstate NY, offers no activities section while any of the other areas I have checked on has them listed,I have asked the staff there why a couple of times but no response!it's nice to know what to expect from a provider, even the ros is sometimes vague!
