Yea, I’d guess the lack of name is the reason for the lack of criteria for PA. Give the Mod her name, have them update it and you should be good to go. Other than that, not leaving anything blank like you did for email is something I never do as well. If no info/email addy, put N/A. I try to never leave anything blank, don’t wanna give them a reason to deny PA for something as simple as that. Just my opinion.
