Encounter Template

Would someone mind sending me a template that has the correct format for are review....thank you...

That is not how submitting encounter reports works. Go to the forum in which you wish to submit a report and click on "NEW ENCOUNTER REPORT". The template will be provided. Then simply complete the form fully and submit.
