Since most transsexuals are M-to-F, pre-op means (s)he usually lives as a woman 24/7, and may have breast implants (or hormonaly induced breasts), but has not had surgery to remove her male genitalia. Most transsexuals that work in the hobby fall into this category, as it is the most popular (fetish-ly speaking). Post-op means they've already had gender reassignment surgery, and are pretty much women with all the expected bits.
