Mine hasn't been used in years. Put a couple of 2x4's across three joists, then long eye bolt through both. The prone "flying doggie" position probably isn't comfortable enough to be of real world use, at least not here. The SO always found it quiet comfortable for missionary.

The height has to be right for the guy, so if a provider had one a quick height adjustment method would be needed.
