I don't think it is necessarily a fetish thing. Some of us are just not as squeamish about female body fluids. My ex and I had sex all the time on her period as she, like Elisabeth, was sometimes very randy at that time. It did not bother me at all, other than it eliminated a couple of my favorite activities. While I completely understand how the cramping and body aches would be reasons for a woman to refrain, I hope embarrassment is not one of them.

I have an IUD so I don't get periods anymore; sometimes I might spot, but the only thing i have to deal with is moodiness and bloating. I do have to say that many gents do not mind a gal being on her period. I actually get really horny and have had only one or two exes not want to do anything. I realize having a personal relationship is different than this, but I am sure, as with everything else, there is a market for gals on their period. Some guys like it and some don't.
Also, when I did have "aunt flo" it was very unpredictable. Mine never followed a regular time so, yes it can be hard to predict when it will come.

I have only had that happen to me once years ago. After I arrived to her incall, the provider took my donation up front and put it away, then told me that her period started and she was not able to do FS, but since I drove all that way, she would do other stuff (Body rub, BBBJ), but no refund percentage. Needless to say, that was the last time I paid upfront!

But, when I did, it happened twice.
The first time we didn't know it until we were done, and yes I was absolutely mortified, embarrassed, etc. I immediately apologized and offered the donation back, he politely declined but I insisted he take at least half back, he declined again so I told him to take what ever he wanted out of the donation and what ever he took back, I offered that amount off our next session should he want to schedule again...and I'm still seeing him as a client today.

2nd time, it hit about 6 minutes after he showed up. I immediately burst into tears because he was from out of town just passing through, probably never coming back to Big D and leaving my place for airport immediately after our appointment and no time to schedule with anyone else. I offered the donation back, he said no and settled with a BJ.
I was very surprised to see he left the entire donation AND left a hefty tip. I called him to come back and get some of it back because it was waaaay to much for a BJ, he declined and told me "hey, I'm married, it happens" but thanked me for not trying to cover it up and act all shocked "after the fact" and could tell I was truly shocked when it happened.

There were a few times I started when I had some appointments pre scheduled, BUT I ALWAYS called and let them know, offered to cancel, keep appt and finish with BJ, keep appt and use dark towels and shower after, etc BUT I totally left the decision up to the client.

And just FYI to the chics who KNOW they are "that time of the month" and act shocked after the session...DONT BE A GREEDY BITCH. It's wrong, it's gross, it's rude AND it's a total lack of respect to the hobbyist. Notify them ahead the minute you start and let them decide....
