I always say "Would you like to use the shower?" and if he says no thanks, I counter with "It would really make everything more comfortable for me if we could shower first" Get in the shower with him and clean him myself. If they don't wanna shower with me, then they can kindly exit.... But I've never had anyone turn down the second offer.
