The WHY is left out intentionally in my methodology of questioning, as many on the receiving end of "WHY?" feel that judgment is forthcoming, if not already in play.
An example: "Why did you do this?" will likely get you an argument and/or a shut down. "I didn't to THAT". etc.
A way of asking the question that invites a more open reception and response is: "Can you help me to understand WHAT was your thought process WHEN you made this decision?" (being careful through one's paraverbals not to imply "this decision" is somehow being judged as wrong or inadequate).
Probably more than you wanted to know, but if you are interested in a further discussion, I have taught at the undergraduate and graduate levels a class in communication skills...
