Is it true that a courtesan will actually take a shower between appointments?
Is it possible to be a courtesan these days?
Is it possible to be a courtesan and advertise for clients?
If you call yourself a courtesan, does that imply you aren't one?
I honestly am not certain. There a few ladies here that I feel are in fact, courtesans or at least courtesan types. And I have never seen then refer to themselves with that term. In some ways, I sort of find it to be like calling yourself charismatic. It is a term to be used by others.
I guess, in short, there are ladies here I would describe as courtesans, but the ones I see using that term, don't fit my ideal. I don't know them, so I could be wrong, but it seems the word has been overused and that takes away from it's true meaning.
When I see that word, I think it's a lifestyle for the woman. Different from an escort, which implies providing a service.
My sig line pretty much describes the sort of woman I seek in this little hobby we share.

Adamant that this is sensual and not to hide belly, etc.

Well, the following only works for certain sci fi fans. But Courtesan ==> Inara on Firefly. She is identified in the show as a 'companion' but I would call her a courtesan.
Urban Dictionary is pretty good on this: A skilled and socially adept provider of sexual intimacy, erotic companionship, and sacred healing, particularly when highly compensated and respected, for example in a court or high society setting
This is similar to what some have already said. High class or high society, high priced (?), adept at the physical aspects of sex but also adept at companionship, including conversation but more than that, advice and understanding. Probably a muse, as mentioned above.

I actually like the term courtesan, per Zhigavo's sig line (i.e.: in the 'classical' sense)...in addition to the physical part of it all I truly like getting to know a woman, and in some sense forming a bond with her, well above simple p4p. sex is WAY better when you start to learn your partner's body rhythms and patterns...but that's just me maybe
i should clarify that : yeah, I agree the term 'courtesan' used in most ads is bs and just a signal for higher rates...its the Platonic Ideal of Courtesan is what I'm talkin about!!
I think there is a real tension between the desire for variety and the desire to 'truly get to know a woman' (quoting Dezperado). Those who hobby make individual choices between seeing many different women and seeing a small number of women more intensively. In economics they talk about investment as deepening or widening, and this distinction seems to apply here as well. Clients decide to what degree they spread their time and money widely versus concentrating it. (It is somewhat interesting that concentrating on a single woman in the hobby has obvious comparisons to marriage, but that is a topic for another time or at least another thread.)
Anyway, I am definitely one of those who concentrates my resources. Deeper, not wider.

I see it as a fancy term for either a mistress or a woman who sleeps around with very selective men/clients. In terms of P4P, both parties are above considering the "relationship" to be a financial one as there is usually some deeper emotional connection involved. Still make no doubt about if, the male does ensure the lady is taken care of.
In today's world, I think it is pretty much a useless term as we are a more egalitarian society. Places where this relationship may still exist are in tribal regions around the world and in places like France/Italy where rich and powerful having extramarital affairs is accepted. Probably the closest thing in the US to a "courtesan" was JFK fucking Marilyn Monroe and Bill Clinton "not" having sex with that woman, Monica Lewinski.
