The rates don't have to be inflated either and my screening practices are hard but worth it for the safety. I know for low volume for me means maybe taking no more than 4-8 appointments a month..

I don't think low volume is an advertising ploy.... For instance when you cater your sessions cook for your clients and spoil them you don't have time to see 5 other guys before him or her in some cases.....
