I'm on a high protein diet...I love special sauce and who doesn't love freshly shucked (with love) oysters.

All of the things you mentioned play a factor, but the three things that bring me back to a provider are;
1. Grooming - The ladies like us to be showered and fresh breath. I expect it return. I'm not made of money and when I fork over a good amount, I don't want bad breath or other things in return. It's a huge turn off.
2. Your attitude - If you are friendly and welcoming it goes along way. While in my head, I know it's your business, the more you do to make it NOT feel that way, the better.
3. Your performance - The difference between a hot provider and a great provider. Even if you don't have model/porn star level looks, if your IOP is off the charts, if you make me feel like I'm rocking your world as much as you are mine, I'll be back over and over again.

Personality is 10x more important than the other two. A little cuddle time and light conversation after the main event just turns me to butter. In fact I think cuddling could be your killer edge.

Personality, likeability is a start. What really motivates me to come back is quality of the kissing. That can be a difference maker in a session. Not only how good the kisses are but do they continue throughout the session right up to and through the climax. And in a perfect world kisses continue after.
How frequently I come back is a function of
1 - how close they are to west side of metroplex
2 - how expensive they are. I am not cheap and blow way more money than I should but have limits.
3 - how long the memories of the kisses last.
