what is the etiquette on using a provider as a reference?

do you ask her or at least tell her you plan on doing so?

It's usually a good idea to give the provider a heads up and make sure it's ok to use her as a reference. Also make sure you have seen her recently within the year.
i dont care if you ask me, i would rather you not.. it just takes more time all together to do so.

if you really want to ask me, just ask me before you leave our date and ill say "of course.. theres no need to email me, just email the lady and as soon as i get it ill let her know that youre fun!"

There is no one etiquette tip because each lady is different. Some want you to let them know in advance that another provider will be contacting them, some don't mind if you use them and don't ask first, some will be ok with it if you have seen them within a certain time period, and some don't participate in the reference process at all. I would recommend asking a provider at the end of an appt what her preference is when it comes to using her as a reference.
