How do I leave a review for a provider?

Not a staff member, but can answer your question. Go to the tab Independent Provider Reviews. Click on that tab, once it opens, look towards the top of the reviews, you will see 2 tabs to click. New Thread and New Review, click on the New Review tab and a fill in the blank form will come up for you to plug in all the pertinent information. Once done, click on Submit and you're done. Usually will take 1-3 days for moderator approval. Good luck
