how do i review a provide?

sorry, been a while, where do i go again to submit a review?

First, go to the city where the session took place (e.g., Dallas), then select the appropriate forum for your review (e.g., Independent Provider Reviews-Dallas).

You'll see a "New Review" button near the top, right above the list of threads. Click on that and fill out the form.
