Different review sections are for different categories of providers. "Independent Provider" means you booked the session directly with the lady, whether on ECCIE, P411, or BP. "Agency" means you used a third party to book the session. The one that can cause confusion is "Other." That's generally for reviews of sessions with ladies you met on Mocospace or Craigslist, or picked up in your car as they walked their beat.
In the appropriate Forum for your session, click on the "New Review" button and fill in each field the best that you can. If she doesn't have email, or a website or ECCIE profile or BP ad, put in "N/A," in the appropriate field, but don't leave the fields blank. If she hasn't advertised a phone number and you don't have her permission to give it out, put in "Provided After Screening" in that field. Fees are generally shown in dollar signs, with each "$" equalling 100 dollars. If you paid 200, you would show the fee as "$$". If you paid 250, you would show the fee as "$$.5", etc.
Because you don't yet have Premium Access you're not familiar with "The Rest of the Story," which is really the heart of the review. Describe the session the best that you can, and be sure to include information that you think other readers would appreciate knowing (things like "she was clean and ready for our appointment," or "her place was a dump and she smelled like last night's lasagne," for example.). But an ROS that basically says little more than "I showed up, we fucked, I came on her tits, and then left" is of no value.
