Back for more! (Returning account to verified status?)

Hiya! I've been away on hiatus for some time now, and apparently my account was downgraded to registered user status as of tuesday this week. I'm hoping to have it returned to verified provider status, as I'm back in town and ready to get back to the fun! (^_~)

Please let me know if there's any additional information, etc, that is needed for this to happen.
