Who can reinstate me?

For the last 6 or 7 months I have not been able to get in to my account. I have changed my password so many times and it still would not let me in. Now after all that time I have been demoted to unverified provider. But I have been here for more than 2 years. Please can someone help me get my showcase back? 214-438-9817

I have reinstated your provider status, if you have no activity for 90 days it will revert back to registered member.
