How do I get my provider status?

I am new to the hobby thing so I hope you fellas will help me out!What do I need to do to get my provider status,if I understand the rules its going to take me getting a review to send to the board.Is that right?Thanks for the help.

Might want to fix the email address in your sig line. Remove the http:// in front of the email.
