Provider returning

Hi there,I was a provider a few years back in Austin and had VP status, but asked to have my account disabled. Is there any way I can get my account active again? I started this account in Dallas a couple months ago and have gotten one review. So it proves I am the same person. Can anyone guide me on how to get my VP status back?
