You need to call dps and see if they even sent the info, they control the PSB and you can complain there. But first you can go online and see if you're listed in the database. This is public information all you need is your name. Your status will be listed if the info has been sent. If you're listed as "active" you can print out that info go to a new company and get a replacement card.
