I've used two agencies, cuteasabutton and SGBJ, neither asked for anything other than send a p411 request. So most they have is burner phone number, and p411 ID. cuteasabutton is long gone, and I don't fear any problems from SGBJ, DA was after the person running it, not the girls or clients.

The problem isn't necessarily one of the immediate recipient's intentions, but of others who may be able to access the recipient's information without the recipient's consent.
Example:
If I'd skipped P411 membership and just gotten started by booking an appointment with NewbieFriendlyNancy, I'd end up giving her my RW info so she can verify through various background checking services that I'm not some criminal with a record of harming women.
Nancy then stores that info somewhere on a computer or in a notebook in a safe so she has some recourse and info to give the police if I go BSC and attack her in session one day.
She might not ever use that info against me, but what if someone cracks her safe or gets into her computer through a 0-day exploit?
At that point, blackmail information could be in the hands of someone with ill will towards me despite Nancy's best intentions.
That's why guys are wary of giving up their private RW info to either agencies or indie providers.
