How to write review

There used to be example reviews, but I guess they are gone.
Go to encounter reports and click on new encounter report
Fill in the info. You have to have a name and contact info.
You have to describe what happened during the date in the ROS(rest of the Story)
Fill out everything and submit it
If you are lucky you will get credit for it
Not too hard!

I do not see how to create a report. Both + icons created a new thread, not a report

Is report creation not available to mobile site?
