where do I go to post a provider review?

Upper right of page select home
Page down to city "Dallas"
page down to Independent Provider review
look for button "new review" upper right next to Moderator

A little extra:
Go the the forum for the city where the session occurred.
Go to the appropriate forum, ie Independent, Asian, Studio, etc.
Click on that forum and click NEW REVIEW at the top.
Be sure to fill in all the info as completely as possible. The more complete the review, the better chance of approval for PA credit.
