It is a legal requirement for massage businesses to keep an up-to-date record of every customer they treated. They all get inspected at least once per year by the health department and must show the record of the customers they served. For example, when you go to your physician, you fill out pages of HIPPA and non-disclosure material. It helps the AMP when you fill these out, so please fill them out, as we want to help the AMP stay in business. They are not asking for your Drivers license, etc. here. As many people have stated, the quality of the information varies, but time and a name is important.
