No premium credit because of "missing contact info"

I completed a review this morning of a provider I saw on Monday. The review is located here:

Just now, I received a PM from surfindick, and was denied premium credit because he says it lacked contact info on the provider.

I just went back, and verified that it indeed DOES have both her cell number, and her email address.
Here it is, copied and pasted directly from my review, just as I submitted it:

We discussed this via PM's, the review was missing url/website information and the provider name was not correct. DallasBenz added the providers eccie contact information and he will be given PA credit. PLEASE guys help us and the provider out by giving us complete contact information as that is how we and your fellow hobbiest find that lady and attach the reviews to their profile.
